Yizkor Book Submissions

Please use the forms on this page to submit the names of loved ones to be included in our annual Yizkor (Remembrance) Booklet and to make your memorial donation. All submissions must be received by Friday, September 4, at 6:00 PM.

Submission Guidelines:

  • Members: Names submitted in 2024 and 2025 are automatically included. Please only use the form below to add new names or if you joined our community this past year. (Suggested donation: $18–$36 per new name).

  • Non-Members: Please submit the names you would like included this year, even if you have listed them in the past (Suggested donation: $36 per name).

  • Need to check past listings? You can download 2024's Yizkor Book here to review who you listed previously.

Note: You can submit your list of names and complete your online donation directly on this page. If paying via check or PayPal, please write "Yizkor Book" in the memo line or Note to Seller.
